Header information

Manifest Tracking Number
Unique identifier and tracking number for a uniform hazardous waste manifest. Automically generated by the e-Manifest pilot system with an e-Manifest pilot specific three-character suffix of WIN.

Manifest Status
The status of the manifest within the electronic uniform hazardous waste manifest process. An e-Manifest begins in Draft status and moves to In-Transit (in custody of transporter), then to Received (in custody of facility), and finally to Accepted (ready for submission to a State environmental agency).

Emergency Response Phone
The area code, phone number and extension, when applicable, used for contact under emergency situations. When supplied, applies to every waste line item on the manifest.

Generator information

Generator name
The company or organization name of the generator.

RCRA Site ID
The unique 12 character EPA identification number of the generator.

Mailing Address
The standard address used to send mail to the generator.

Site Address
The physical location information of the generator.

Contact
The area code, phone number and extension, when applicable, used to contact the generator under non-emergency situations.

Email
The contact email address of the generator, when applicable, used for contact under non-emergency situations.

Transporter(s) information

Transporter sequence number
The position of the transporter within the list of transporters. Transporter sequence number will begin at 1, and incremented for each additional Transporter.

Transporter name
The company or organization name of the transporter.

RCRA Site ID
The unique 12 character EPA identification number of the transporter.

Designated Facility information

Designated Facility name
The company or organization name of the designated facility.

RCRA Site ID
The unique 12 character EPA identification number of the designated facility.

Site Address
The physical location information of the designated facility.

Contact
The area code, phone number and extension, when applicable, used to contact the designated facility under non-emergency situations.

Email
The contact email address of the designated facility, when applicable, used for contact under non-emergency situations.

Waste Details

HM (hazardouse material)
Indicates whether the waste line item is hazardous material or not.

DOT Description: Proper Shipping Name
The Proper Shipping Name of the materials designated by DOT as hazardous.

DOT Description: Hazard Class
Hazard class or division used to reference the requirements for packaging, labeling, and any special requirements that must be met by generators and transporters.

DOT Description: UN/NA Number
United Nations (UN) or North American (NA) number used to identify hazardous chemicals or classes of hazardous materials.

DOT Description: Packign Group
The relative degree of danger presented by the hazardous material.

DOT Description: Emergency Phone
The emergency contact phone number for the individual waste line item. Used in cases where the Emergency Phone indicated by waste generator does not apply to all waste line items of the manifest.

DOT Description: Other Description
Additional text describing the DOT Description of the waste item.

Container No. (# of containers)
The number of waste containers for the waste line item.

Container Type
Code identifying the type of container used for hazardous waste shipment for the waste line item.

Total Quantity
The total quantity of waste for a given waste line item, in unit of measure indicated by Unit Wt./Vol. type.

Unit Wt./Vol (Weight/Volume)
The unit wt./vol. codes used to quantify the total waste quanity on a waste line item.

Waste Codes
Codes of hazardous waste types that may be used to identify waste in a hazardous waste shipment. Includes both federal and state waste codes.

Special Handling Instructions and Additional Information
Additional information or instructions for proper management or tracking of the waste.

Generator/Offeror Certification

Printed name
The printed/typed fullname of the signatory for the generator.

Signature
The electronic representation of the signature. Will display "Electronically Signed" in cases where the generator utilized a PIN for electronic signature, or in cases where a mobile device was utilized, an image of the generator's physical, digitized signature will be displayed.

Signature Date
The date and time the signature event occurred.

Transporter Acknowledgement

Printed name
The printed/typed fullname of the signatory for the transporter.

Signature
The electronic representation of the signature. Will display "Electronically Signed" in cases where the transporter utilized a PIN for electronic signature, or in cases where a mobile device was utilized, an image of the transporter's physical, digitized signature will be displayed.

Signature Date
The date and time the signature event occurred.

Discrepancy information

Quantity indicator
Indicates whether a quantity discrepancy exists on the manifest, or not.

Type indicator
Indicates whether a Type discrepancy exists on the manifest, or not.

Residue indicator
Indicates whether a Residue discrepancy exists on the manifest, or not.

Partial Rejection indicator
Indicates whether a Partial Rejection discrepancy exists on the manifest, or not.

Full Rejection indicator
Indicates whether a Full Rejection discrepancy exists on the manifest, or not.

Discrepancy Description
Supporting information further describing the discrepancy(s).

Manifest Reference Number(s)
If the event of a residue or rejection occuring on the manifest, where a new manifest must be generated, this will contain the manifest tracking numbers of the manifest(s) resulting from residue or rejection discrepancy.

Alternate Facility name
In the event that an alternate facilility (or generator) is specified, this is the company or
organization name of the alternate facility or generator.

RCRA Site ID
The unique 12 character EPA identification number of the alternate facility or generator.

Site Address
The physical location information of the alternate facility or generator.

Contact
The area code, phone number and extension, when applicable, used to contact the alternate facility or generator under non-emergency situations.

Email
The contact email address of the alternate facility or generator, when applicable, used for contact under non-emergency situations.

Hazard Waste Report Management Method Codes

Management Method Code
The hazardous waste report management method code, used for biennial reporting, for the given waste item.

Designated Facility Certification

Printed name
The printed/typed fullname of the owner/operator signatory for the designated facility.

Signature
The electronic representation of the signature. Will display "Electronically Signed" in cases where the designated facility owner/operator utilized a PIN for electronic signature, or in cases where a mobile device was utilized, an image of the physical, digitized signature will be displayed.

Signature Date
The date and time the signature event occurred.
